- Your diagnosis, in writing, from your doctor
- Recent reports: blood tests, scans and biopsy results, if you have them
- A list of your current medicines
- Your budget, including travel and the attendant
- The dates when you can travel
- Questions for each hospital (see each treatment guide)
Do not send full reports to us. Send them to the hospital that replies to you.
